His dad pushed him out of a 8th floor window when he confronted him about his dad's wedding. His dad denied killing his soon to be son-n-law and became irritated with Locke and pushed him and he broke his back.

My guess is that it was some sort of a fall. From the previews for this episode, he looked pretty banged up.
