People who saw the fastest pitchers ever,Feller,Ryan,Johnson, all agree on on the fastest ever.Steve Dalkowski.Name doesn't ring a bell?He never made it to the majors. He could throw as hard as heck but he couldn't controll it.It was estimated at 110 mph.Ted Williams said he threw the fastest ever and he played against Feller. The fastest in an mlb game was by Nolan Ryan at 100.9 mph.

Heres a site

Different radar guns give different readings, but the fastest pitches recorded have been in the 102-103 range. But anything in three digits is smoking.

Of course, no one was clocking Walter "Big Train" Johnson back in the day, so it's hard to say how fast his pitches came in.

Fastest is interesting but control with high speed is where the cake is cut. Sandy Koufax in his prime could throw a mean fast ball and come back with a curve almost as fast. I recall him striking out the side in the World Series with the Yankees, Mantle just laid his bat on the plate and walked away after watching the ball go by.

Mark Wohler in 1995 spring training he threw it 103 mph.

Here is something to think about though...Second link in my sources, it shows what could of happened back then ;) and why it is so hard to say who the fastest actually is.
