I don't know what level four means, but I have played piano since age 5 and clarinet for the past 7 years. When I practice, I play things slowly and speed up the tempo until I can do it at the right tempo. This can take anywhere from 15 minutes to 4 hours. However, after a while, you start getting diminishing results. The rule I follow is 3 times perfectly at a reasonable tempo (faster than the day before if I'm increasing it). You play a song as many times as you need to to feel comfortable with the notes, rhythm, style and tempo. Scales you do until they are flawless (of course), but this takes time. What I try to do is go through all the scales chromatically (if you start with C major, do C# next, and so on), but I start over every time I make even a tiny mistake. So, you see, it's not really a matter of the time you practice (although that's great for bragging), but the quality of the time you do put in. You can improve if you spend just an hour every day focused and practicing efficiently. Even a half hour can be productive.

7 to 12 hours a week on practicing and 2 to 3 on scales. Always play scales before practicing at least 10 mins.

I am a musicians daughter and let me tell you this: if you want to be a really good musician who enjoyes playing for the rest of his/her life, you practise as much as you can as often as you can until the things you practice go by themselves and take no effort whatsoever on your part. then you move on to the next excersise... just have fun with playing and getting good!!
