is it worth it to spend that extra money on premium or use regular?

This was designed to run on 87 octane. Any more than that would be a waste of money.

Run regular in it. Remember that the computer adjusts itself for your driving habits as well as the fuel. If it only makes a little noise (pings) give it a chance over a tank full or two. If there is no reduction in the light pings, then go up one grade.

With a proper tuneup, your '91 should be just fine on regular.

If you have a V6 you could get away with 89 octane. If you have a V8 you could do a test with 89 octane if you drive the car the same way for one tank full checking you mileage, then use 93 octane and check your mileage, but this will only work if the car is well maintained, If you hear pinging with the 89 octane under a load then don't bother doing any testing.
