There is a freestyle relay and a medley relay, not a freestyle medley relay. Below are the records set at the college level as well as world records below that. I haven't found a location for high school level records, since most are at the individual state level.

NCAA womens records
3:13:56 - 4x100 y freestyle relay - 2005 Georgia
3:31:45 - 4x100 y medley relay - 2002 Auburn
3:35:14 - 4x100 m freestyle relay - 2004 Georgia*
3:56:48 - 4x100 m medley relay - 2004 Georgia

World womens records
3:35:94 - 4x100 m freestyle relay - 2004 Australia*
3:56:30 - 4x100 m medley relay - 2006 Australia

Not sure why the 04 NCAA time is not listed as the world record which is .8 faster than the 04 Australia time.

The womens long course FINA approved world record (www.FINA.org) is:
4 x 100 free 3:35:22 by Germany in July 06
4 x 10 medley 3:56:30 by Australia in March 06

The womens short course FINA approved world record (www.FINA.org) is:
4 x 100 free 3:33:32 by Netherlands in April 06
4 x 100 medley 3:51:84 by Australia in April 06

OF course there are FINA World Championships on at Melbourne, Australia this week and next .... so you should expect these long course records to be blown away (well we will see)!
