Actually, you can do it both ways. Most peole stretch by using a taper to go up a size every month or so. Any faster than that, the hole will not heal and you run the risk of ripping the skin. You can get tapers from a shop and do it yourself, or go to a professional and have them do it for you. Some people also just shove bigger gauges in their ears, but this isn't recommended. Doing that can tear the lobe and/or cause the hole to be crooked.
The other way to do it is with a dermal punch. This actually is similar to the idea of a hole puncher (but much safer). This has a longer healing time, but can skip a lot of waiting periods between gauges. This also is much harder to heal up if you ever want to go back to "normal" sizes.
There is also a small community that uses a scalpel to cut out a hole. Many people will go to a doctor to do this, though some piercers have been known to use this method. This can be used to cut down on the wait time, and because it may seem less of a shock than a dermal punch.

DO NOT use a whole puncher , that person was just joking ! I hope anyway ! It is called guaging. You put in a bigger size like every two weeks so it is done gradually. My cousin had that done . If you go to a piercing place to have it done I'm sure they would tell you everything you need to know after that ! I personally think it's ugly though ! If you are not wearing earrings or EVER plan on taking them out , you would be able to see right through the holes, and it will NOT grow in like a regular piercing ! Think twice before doing it , but if you do it , do it right !
