are you asking how it happens or how to achieve it?
I would say it happens as a natural response. We can all relate to an accelerated heart rate when something has happened, or is about to happen, so playing the music in the background of a "thrilling" moment enhances to us (to our ears-to the brain) what is happening to us naturally in the chest.

All that is, is a common form of subliminal advertising that the audience is not suppose to notice. The goal in the heartbeat scheme for horror, thriller,or suspense etc.trailers is to actually make the audience think that the pounding heartbeat is their own or at least enhance the feeling of ones own thrill by adding that thumping heartbeat sound. It's just a form of planned advertising to give the product maximum appeal to bring in the crowds that got moved by the trailer's impact.

Because people think that when they hear a heartbeat background either someone is going to die, or something exciting is getting ready to happen.........my opinion.
