It's either over fed, constipatied or has an internal infection. Can't really say which from what you give us to work with. Try not feeding the fish for a day or two and see if he gets better. If not it's an infection and you will need to treat that to try to save the fish.

If it looks spikey it may have an internal infection known as Dropsy. This will kill it unless you get something from the pet shop. There is a product called Anti Internal Bacteria on the market and it does the job.

Could be, sometimes fish 'swell' when they are on the way out.... either that or pregnant! lol
