Stay out of her way, avoid her if you run into her, that's the best way. It's not like sending her flowers is going to make things better.

Don't dredge up old wounds. She's probably gotten over it and wouldn't want to be reminded of the past. Saying sorry would make you feel better, but make her feel worse - the time for apologies has passed.

Let's face it - good men are hard to find and all's fair in love and war.
Friends rate a distant second.
Nice guys finish last.
Let's face it - if you took him away from her either he wasn't in love with her and was just biding his time to get to you in the first place (yes, men do that) or he isn't worth sticking with.
I hope you got what you deserved.
That crazy game called Love!

if you're truly sorry, i'd say the best way to get your point across is to write your thoughts to her in a letter. then try to meet her for lunch or something to discuss the letter and any other issues that may arise. best wishes!

just tell her that back then you were a different person and that you are very sorry about the way you acted and wanted to be friends now. tell her that you have changed. atleast you can say you tried even if she wont forgive you.

just say your sorry, I guess. Tell her you were a different person then, and you have regrets for how you handled things then. (if you actually do think you should have done things differently)

what good would that do now? does she already know? if not, you will just be causing her hurt. if she does know, and still speaks to you, she let it go. you should too. if you need to "confess" to relieve your guilt- thats your problem. you hurt her once- leave it be.
