"Professor" is the only word that needs to be capitalized; "lieutenant" and "doctor" do not. Even though they are a persons title, rank or position, when used alone, or even when used after the person's name, they are not capitalized. They are considered common nouns or adjectives unless they immediately precede a person's name.

FOLLOW-UP: Actually 1st poster may be right. I did find rule that allows you to capitalize a title without it preceding a person's name as long as you are directly addressing them, which is actually how I usually do it, too, but it's always good to double-check yourself and learn new things!
