"My old lady" reminds me of a bag woman. I'm 28 and would hate to be referred to as that. I think the same of "My old man"--some wrinkly old man sitting in a bar somewhere smoking a cigar. That's just me though:)

At times, yes, jokingly.
In my realm of friends, there be a few hard-core Harley fans, who live a certain lifestyle, they tend to use that expression. I don't like it, because my wife is young & hot, but it does not bother me or her.

I would only do that in a humreous situation, most likely with no one else around but her. Never in causal reference, or in everyday usage.
