Is the thing that separates a fangirl from a fanboy their gender, or are there more differences between these two ways of expressing your fandom?

Other than standard connotation, it is only gender. But into stereotypes:

Fangirl: Unknown weight and smell, usually seen in Sailor Moon T, while heterosexual, believes all anime characters gay or lesbian, except for their favorite of the opposite gender, who is strait and the only man who will father her child

Fanboy: Overweight, stinks, Wears sailor moon T-Shirt or Faye Valentine costume. Think he's Tuxedo Kamen.

Fan girls are a lot more easier and common to find than fan boys.

Sometimes you can tell the diiference by looking at their avatar and see if the image is more female or male and then you can work it out from there.

Also you can tell by the usernames they use. If the name sounds more masuline or feminine.

There are the obvious physical differences too.

But when it comes to fan art or fan fiction, you can't tell the difference.
