Depends what you mean by "visited" the Moon.
Twelve men have walked on its surface as eggman has detailed. Each of those 6 missions had a Command Module Pilot who orbited the Moon while his colleagues went for their excursion.
You can also count 3 men in Apollo 10 (Tom Stafford, John Young & Gene Cernan) who made their voyage in May 1969 as a dress rehearsal for the first lunar landing, and Frank Borman, Jim Lovell & Bill Anders in Apollo 8 who orbited the Moon in December 1968.
I make it a total of 24 who've been in the vicinity, including the 12 who actually landed.

Adding to Whoosher - a total of 9 missions made the trip to the moon, and 3 of those guys went twice:
Lovell, Young, Cernan.
Whoosher's right - 24 guys have made the trip, and 12 stepped foot on the moon.
And the last guy *set foot* on the moon was Schmidt (Apollo 17), but he got back on the LM first, leaving the *very last footprint* on the moon to be made by Cernan.

12 men have actually walked on the moon.... each (successful) Apollo mission had a 3 man crew, but one guy stayed with the capsule orbiting the moon while two went for a walk. 6 Missions actually made it so that's 12 guys (not 14).
