Chromosomes are the complete packaging along with the DNA. In some organisms, the chromosome has very little protein packaging and is mostly naked DNA, in others, it is densely packed.

Chromatin, on the other hand specifically refers to the densely packaged DNA that contains histones, which are proteins that the DNA is wrapped around for efficient packing. While chromatin contains both protein and DNA, it refers only to the areas of the chromosome that are densely packed, while the chromosome is the entire thing, even the less dense areas where much of the transcription is taking place.
