Scientists believe that the moon formed as a result of a collision known as the Giant Impact or the "Big Whack." According to this idea, Earth collided with a planet-sized object 4.6 billion years ago. As a result of the impact, a cloud of vaporized rock shot off Earth's surface and went into orbit around Earth. The cloud cooled and condensed into a ring of small, solid bodies, which then gathered together, forming the moon.

The most plausible hypothesis is that earth collided with another planet or big protoplanet.the collision resulted in the
ejection of very large amount of rocks into space. After orbiting the earth for several million years they attracted each other to form the moon.At first the moon was closer to earth than now and is moving away by 4cm per year.

there were a few different theories.
1) Its was originally from the pacific ocean. Not very long after the earth was born, when it was still a big piece of molten rock, the force from the sun had made the earth's self-spin faster. therefore, some losen piece of rock had been "swing away" from the mother earth. it was the moon, and the hole left behind became pacific ocean, which only named arnd 150 ~ 200 yrs ago.

2) Very typical, it was form as same time as the earth.

One theory; the earth "gave birth" to it when it was hit HARD by an asteroid many moons ago. The impact dislodged enough stuff (1/5) to eventually coelesce into one big rock, because of their mutual gravitational attraction.
