Pompeii on Nacogdoches

or

Baccone's on West Avenue.

Of course if $$$$ is no object; then Paesano's off 1604 near Stone Oak Parkway

Paisono's has high ratings, but I don't think that is one I have eaten at. We visit San Antonio from time to time and found an excellent Northern Italian restaurant with great wine list hidden in a distant corner of the walk next to a large hotel (unfortunately I am terrible with names). Looking on Google, I think that it is most likely Michelino's at 521 River Walk. You being there , you could check it out—but it was genuine Italian (we live in Chicago). I'm sure you are also familiar with Boudro's (not—Italian but I know that name for sure) which has an excellent lunch and they make fresh guacomole table side. I hope to be there again soon and revisit these places and write down there names. Hope you enjoy..

There is one in San Antonio that has to be the Best Real Italian Restaurant anywhere! They are authentic and do not mess around when it comes to the real deal. This is one fine place and serves up a fantastic Italian meal. The Place?

Little Italy Restaurant
824 Afterglow St.
San Antonio, TX.

I use to live in San Antonio and down on the Riverwalk is this fantastic Italian restaurant. I can't remember the name but it's almost at the end of all the shops. It's the best.

Little italy is nasty and dirty. The best Italian food in San Antonio is Grissini in the med center. No comparison.

San Antonio express news critics choice, and my own personal favorite. Its so much better than the others its ridiculous.

Grey Moss Inn is my vote... Tower of the Americas has the best view, but the worst service. Didn't much care for the food either! The Vineyards is also nice (but not technically in San Antonio)

Lorenzo's at the corner of Fredricksburg and Medical. Small with great atmosphere and better food.

Napoli's in Universal City on Kitty Hawk. Better avoid this place if you're on a diet. The garlic rolls are praiseworthy and the food is richer than Donald Trump. I'm drooling just thinking about it.

They don't! everything has a damn jalapeno in it! Girl if you want real Italian food you've got to come to New York/New Jersey! I was born and raised here. I live in Virginia Beach for 2 years and Austin, Texas for 7 and hauled my @ss back to New York! This is where the real Italian cuisine is.

I do agree with several of the previous answers. Little Italy is very taste. Pisanos is absolutely fabulous. There are multiple locations. Do not go to the far north 1604 location, bad service. The location by the quary is the best. Be sceptical of many restraunts on the river. My office is on the river and they are often visited by local wildlife (be afraid).
There is a restraunt on 1604 and stone oak by the name of Reggianos. It is top notch, pricy, and the bar attracts many mid aged women dressed to kill and thrill. You will not be disappointed.
