You are required to be given at least 30 minutes, off the clock, lunch break when you are scheduled for more than six hours. You must get two 30 minute off the clock breaks when you are scheduled for 12 hours. You are also allowed a 15 minute, on the clock break for every four hours that you work.

Yes it is. You should get at least an hour for 10 hours. It's up to them how it's split up though.
If it's a private organisation people can be a lot harsher. I work in a private hotel and i sometimes get half an hour for doing up to 14 hours a day.

No, you should get an hour lunch as well as two half hour breaks. Unless they are paying OT.
