If you have any plants with points or sharp edges in your fish's bowl or tank, it could be a rip. The good news is that your fish is probably okay, and the tear may even heal. Keep an eye out for strange behavior - darting around the tank, lethargy, refusing to eat, etc. - just to be sure. Also, pet stores sell water additives that can help your goldfish heal faster, so you may want to look into those if it doesn't improve within a day or two.

The question really doesn't have enough info to go on. Can you tell if this OPEN gill is red or have a particle in it? Most gold fish will "suck bubbles" in order to dislodge anything that may get stuck in their gills. This is normal behavior for gold fish. Especially if you do not see reddening or object stuck.
It is a possiablity he had gotten it stuck on something in the tank, although highly unlikely. If he isn't breathing heavily, swims normally and eats, I would just keep an eye on him. Doesn't sound too serious at this stage. One of the things you may want to keep an eye out for is any reddening or whitening around that gill slot.

What changes. He is struggling in the polluted water for oxygen. Remove 20% with a gravel vac, two days in a row. I don't know if it is in a a tank (I hope so) and not a bowl. Use a dechlorinator for the water. If it is in a proper tank with filtration, you may also consider putting aquarium salt in at the rate of a rounded tablespoon per 5 gallons. Don't use table salt.
