Phase change refers to a change between the various phases of matter, solid, liquid, and gas. Evaporation of water is an example of a phase change. Chemical phase refers to which state of matter a chemical is in.

Phase change is changing of liquid to vapor and solid or vice versa. Chemical phase depends on ions in it.
