though rare there are women who die during or after childbirth--Some causes:

Severe bleeding (hemorrhage)
Severe anemia

amniotic fluid embolism (1 case per 8000-30,000)

Pregnancy-induced hypertension (preeclampsia and eclampsia

Infection during or after labor
(sepsis)

aneurysms

Low birth weight of the child increases the risk of maternal death from cardiovascular disease. Subtracting one pound of infant birth weight doubles the risk of maternal death. Therefore, the heavier the birth weight of child, the lower the risk of maternal death.

I dont have updated numbers but the occurance is low

The last figures I found for the US (about 5 years old) was 11 deaths in 100,000 births

If women die during childbirth it's usually as a result of some other underlying problem. It is extremely extremely rare to die during childbirth nowadays. It's possible to die from hemorraghing after giving birth but that's also rare and more often than not doctors are able to stop the bleeding.
