OK -- I will give a serious answer -- only because an internet search may come up with something that you shouldn't see.
It has two meanings. It is used as an expression indicating displeasure with someone -- roughly equivalent to the meaning of "Go to hell" or "You are a jerk."
Its second meaning is the more literal one -- it refers to a sexual act involving contact between the mouth and the genitals.
