If a friend rents a car and then lets me drive it without telling the car company, is that a big problem? What would they do if they saw me driving?

The company wont necessarily do anything. If the police catch you it is like a stolen car. You are not authorized by the company to drive the car so it would be like you stole it. As long as there is no damage and you dont get caught by the police the rental company wont pursue the issue. If you are caught it may put your friend on a DNR (Do Not Rent) and he may not be able to rent a car again.

Then the insurance he bought would have an excuse not to pay. Some rental agreements specify who can and who can't drive the car. Usually it is an age requirement. Don't mess up your buddy, and get him into trouble, let him drive.
