I remember that Miss Manners answered this years ago. She said It is really just a matter of preference. If you go to the nicer hotels though you will notice that the paper is always loose end over the roll (sometimes they fold it into a little point).
The drawback to this style is that, if I leave the bathroom door open, my dog likes to "play the toilet paper game" by taking her paw and spinning/shredding a half roll of toilet paper. If we put the paper on "backwards" (ie: beneath) the roll just spins and the paper doesn't come off the roll.

Depends... I've always hung it with the loose end coming off the top because it's more convenient (It's also how I was trained at a hotel I worked at once.)- easier to get hold of... but I've seen that backfire.
I knew a little child once (toddler-age) who happened to go in the bathroom one day and slapped at the TP, which responded by starting to come unrolled. Of course, the kid thought that was a great game, so he kept doing it, then he decided that big white thing with water in it was another great toy and started flushing the unrolled paper.
I don't remember if the whole roll ended up getting flushed or if he ended up clogging the toilet, but his mother wasn't a happy camper!

Above. I worked for a professional cleaning service years ago and if you put the rich, snooty peoples' toilet paper the wrong way, god forbid! It turned into a pet peeve..I gotta have it the right way!

Above, the loose end should always be on top... not directly but hanging on top. Its the proper and classy way to hang toilet paper.

Beneath the roll, closer to the cabinet, or the wall or whereever you have it is better; it is more luxurious, better looking, and won't get ripped or pulled on when you pass by it. This method is used in most Las Vegas hotels, and other luxurious hotels.
