Georgie is right...douche bags were used by women to clean their nether regions in a process called douching so to be called a douchebag or a douche is meant to be derogatory due to the connotation...

A douchebag is use to rinse out ones vagina or anus. That's why it is offensive I suppose. It's a pretty lowly thing to be, ha ha
