Have a friend who knows what will be on the road test go with you on a practice test (or several practice tests). Your friend should pretend he is the Motor Vehicles officer (even with a clipboard), and make you take your (practice) road test -- as many times as it takes until you get comfortable with it.

Just take a few deep breaths and relax. Sounds like you're a pro already(15+ years). Just do what you always do. Make sure you study for the test. Best bet...if not too late... take a driver education course so that you KNOW what you'll be tested on. Always use your directional when you turn. Make sure you come to COMPLETE stops at stop signs. If all else feels, slip the guy a $50 bill(unless it is illegal, then DONT do that)!!

Get some beta blockers at the doctors office. Just tell the doctor you have this really big presentation in front of 100 people at school and you would like to appear not nervous. Ask for a beta blocker. It is a blood pressure medication, but it makes you appear to be more calm. Trust me, this is what you want. You may still feel nervous, but you won't look nervous. People take these for this purpose all the time.
