Click on his page.
Scroll down quite a bit and you will see letter template there.
There are several templates - just in case first letter doesn't do the trick with your bank.
It's a step by step explanation on what to do.
Good luck with your bank - don't forget to keep copies of the letters and dates you sent them!

too many charges, well make sure you have their contract as a resource-so you know why you got all those charges, maybe they were fair because you didn't read the small print, go over your bank records...
