There are many instances where the Accio charm was used
- Accio Firebolt during task 2 in GOF
- Accio Cup in the graveyard in GOF
- Accio Rosemerta's brooms in Hogmeade in HBP
- Accio Horcrux in the cave in HBP
The last one was the unsuccessful one...it showed Dumbledore that Inferi were guarding the Horcrux, though

The charm is used throughout the series
The important times were:
accio broom in book 4 at triwizard challenge
accio portkey in graveyard in book 4
accio prophecy in book 5(unsuccessful here, the caster was blocked and did not get the object of desire)
accio horcrux in book 6(unsuccessful here, object of desire too well guarded)
There are many other instances in which the charm was used. I guess you could count the times when the students were learning the charm and were unsuccessful in casting it effectively?

1. To call his broomstick when he is fighting a dragon (Goblet of Fire)
2. To call the portkey (Goblet of Fire) while he is in the graveyard with Cedric and Voldemort.
--They were both successful in that they did what they were supposed to do (bring something to the caster).
