I have a 18" Neck and 36/37" sleeve. Usually if I buy these off the rack, they fit horribly. The waist and chest is way to big. Calvin Klein used to make a slim cut Dress shirt that fit well. Any other ideas??

John Varvatos cuts very small. Also Zegna cuts small, but the prices of both cut rather large. Also Marc Jacobs definetly runs narrow.
