Too much pressure to succeed. Too much worry. Too much "trying".
When your focus is too much on the completion of the goal (i.e. getting good grades, finishing a project in a certain time, etc. etc.), not in doing the task, you can lose your way and hinder your own progress.
Set your focus on small goals and find some joy (or at least interest) in what you're studying. Then, the success you seek will come.

Read a good biography of the Late President Abraham Lincoln of the U.S.A. - get a book which specially brings out the failures he had, as compared to his successes. Even when, he did become one of America's most successful Presidents, handled the American Civil War successfully, and went on to have, perhaps, a happier and quieter second tenure, he was shot fatally in a theater hall...If you read about his life, you will surely understand, what power lies within a determined human being, and how he "converts" impossible failures, into outstanding successes...

See a doctor to make sure you don't have any underlying medical conditions causing these problems. Otherwise try someone like a life coach or a similar profession to help get your life back on track.
