When they did the pilot for South Park, they killed Kenny at the end of the episode as a joke. When the show was picked up and they showed the second episode to the producers, they kept asking them where the little kid was who was wearing the orange hood. The network people really liked Kenny and wanted him back.
So, they wrote him back into the episode. When they were trying to decide how to fit him in, they gave him his part in the show, and thought "Well, we have to kill him again!" It was such an odd idea, that they decided that he would die in every episode.
