because we all have kids) another couple are always invited. We don't dislike these people, but don't have anything in common with them and generally don't enjoy being social with them. We also feel like the 'third wheel' when they are around because they spend a lot of time with them. Should we say something or just keep politely declining invites when it is us 6 present?

Instead why don't you try inviting your Sister-in-law & spouse out so there is no chance of the other couple coming. When they call say well why don't you & your spouse come to our house & send the kids to friends or grandparents houses. Then the other couple won't have any reason to come cause they aren't invited & it's not a public place so they don;t have any reason to just show up, or make it a family thing & let the kids play while you visit with each other.

Why not do an invite to go out with YOU---then it will be just the way you want it. They have the right to invite anyone they want--and you have the right to decline the offer--but everytime you pass up the opportunity, you don't take the time to get to know something about these other people---you just may like them as time goes on.....well, it is your choice. It is tacky to say something because an invitation is like a gift. You dont criticize a gift someone gives you. Instead just decline.

Why not invite them out so you dont have to worry about that other couple?

I believe you should ask you brothers sister in a polite way if all you guys could go out exclusively without the other couple.

Its nice been invited, but if you feel uncomfortable then once in a while you can decline....they will understand

Seems odd that they need another couple when you're there. Almost like "boredom insurance". Have you asked them about it?
