One becomes an MD (medical Doctor) first then specializes...family practice where your patients come for regular complaints and the annual checkup and those kind of problems...An Internal Medicine Specialist takes care of Internal (surprise )organs.. and often has a sub specialty in kidney , liver, pancreas etc.

I work for an Internal Medicine doctor. The difference is that they don't treat children. They deal mainly with patients who have diabetes or thyroid problems.
