For a single violation, you lose 10 points (top contributor or not)
If Yahoo deems the violation as very serious, or if you get a lot of violations, your account may be suspended (which really means terminated) - then you have to create a new one and start over.
Most of the violations and suspensions are for very minor things. It's ridiculous.

I do not know. I did report one of them who was very obscene some time ago.
