In the reclining position, the hydrostatic pressure (pressure of a column of fluid) is minimized.

When the patient stands, then there is a hydrostatic effect. There is also some degree of pooling of blood in the distensible leg veins. Hence the upper body (incl arm) blood pressure drops.

However, in young fit and healthy subjects there are a set of reflex mechanisms to adjust for this. Once these kick in, there should be very little difference between the lying and standing blood pressure.

These reflexes include the initiation of a tachycardia - increasing the heart rate to increase the blood pressure. Thus you may find that heart rate will increase when the patient stands up from recumbent position.
