Breath support! Learn some good breathing techniques. It's easy to practice and helps your voice and voice control. Breathing is the single most important thing in singing.

It sounds like you're are probably singing in your throat too much and aren't using proper breath support and perhaps forcing yourself as well.
I would recommend lessons. If you can't get them then search on line for websites that explain proper breathing while singing.
I have a book ; 'The Vocal Performer' by Jeffrey Foote, that I think explained things pretty well. You could try to get a copy of that.

I have the same problem, some things I've tried are breathing exercises, pitch exercises, opening my soft palate more, and staying away from anything dairy before singing.
That should help, but your voice still might crack depending on how long the note is.

... but anyway, practice with a vocal coach...
