46 is normal but there is a problem called nondisjunction which occurs in meiosis and mitosis. Examples of this is Downs Syndrome where a person has an extra number 21 chromosome. In the case of 48 chromosomes this occurs many time dealing with the X an Y chromosomes. You can have individuals with XXXY or XXYY arrangment. This is called Klinefelter syndrome.

yes many do. its the extra sex chromosomes and it results in diseases
