Its not something that happens over time, like from 'weeks with no sex'.
Its something that happens on a specific day if there's a lot of arousal and then no release. It does not always happen. Its caused by fluid buildup in the area (not semen actually) and can cause a severe cramping feeling from the inner thights up to the abdomen.
Women can actually get it too.
Ejaculation helps get rid of it, however its triggered (for men).

The term blue balls is used to describe the dull ache in the testicles that a man gets if he is extremely aroused but is not "permitted" to orgasm... that is.. when he is brought nearly to the point of orgasm and then left hanging without joy. It is a physical manifestation of the disatisfaction experienced. The only way to provide relief is to orgasm by whatever means are at your disposal.

Well no, the testicles don't turn blue. Like the others stated the pressure of semen build up causes a man to feel a little pressure, pain, uncomfortableness down there.

Blue balls is when a guy needs to *** REALLY bad and can't cause no one turns him on enough. It's really bad for guys cause it can really hurt them badly. Some people have been known to die from it or suffer mental and physical injuries. It's pretty bad really. but good when you want like and ex to suffer or feel your pain. lol. it worked for me.

mostly it's fiction like the boogie man. something guys say when they haven't gotten *** in awhile and want to guilt their girlfriend/date into having sex with them or giving them head. but when guys don't release themselves either through sex or masturbation they do have a slight pain from all the buildup and no release but i['ve yet to see them turn blue.

Well, as I know it, it's when a guy hasn't had sex or masterbated in a while and the pressure builts up and it hurts.
It's a dull pain, but very noticable.
