Insanity can only be diagnosed by a psychiatrist.
The person would have to have a continual disconnection with reality. The person would have to be so far from reality and normal activity that they would be unable to function basic human behavior. They also might be a danger to them self or other people. They would have a diagnosis acknowledged mental disorder.
The institutionalization of the insane during the 1970's has put many people on the street who need to be taken care of.

what makes one insane, well theres a problem with the question how do you define insane. but generally theres many things a supressed memory of a bad experience is supressed to allow the body to cope and to cope the person may do things classed as insane. Faulty thought processors. mis balance of chemicals in the brain. but i think mainly supressed memory, if a person dies horrifically you may be traumatised to a point you dont move and rock all day this may be seen as insane

I think probably a psychiatrist can only legally declare anyone insane. Generally it's just labeled as a mental illness. I think possibly the best definition I've ever heard was by Albert Einstein who said: "Doing the same thing over and over again and expecting different results."

not being able to perceive, comprehend, and put into practice what is something real. Something real is what is perceived by the mind, it has form and matter and most, being experienced by the senses. Otherwise thinking about a thing that doesn't have this three qualities is obsession, leading to insanity.
