Births and deaths are Provincial records... check with your Province's web site? Otherwise try Canada's website...

Not sure exactly what you're looking for...are you looking for a claim form for death benefits? If yes, from which Canadian agency? Canada Pension Plan offers a death benefit if the deseased has been paying into this fund....but other agencies may have separate death benefits....
