I'd shake their hand if I didn't know them well and they might be offended if i didn't (i don't think you'd shake someone's hand if you knew them well anyway, but never mind!). Afterwards, i casually run to the nearest sink or person wearing fur (!) so i could wipe and wash my hands

I would nod and say how do you do,or nice to meet you, but not, under any circumstances, shake the hand that is being offered.
