Go to your local social security office and fill out the application. Bring your ID, like your birth certificate and your photo ID and they will give you a letter showing that you applied and you should recieve a new card in about 2 weeks.

Go to the Social Security office and ask them to 'make you' a new card ... they'll do this for free so all it will take is a bit of time ... and the 'wait' for the new card to come to you in the mail.

You'll have to go, in person, to the local Social Security Administration office.

You'll also have to take several forms of ID with you. And though you don't say anything about what happened to the old one, you might want to check your credit reports if it's missing.
