Contact your family doctor and she/he will have them sent to you. Of course there may be a fee involved but you have all rights to these documents.

As the UK is a participant in the visa waiver program you should be able to cross the border into the US without much trouble. You will want to make sure that you bring your work permit with you as it may be required for your return to Canada.
