Painful sex or dyspareunia, volvodynia and pelvic pains.?
Pelvic and sexual pain can be caused by physical or psychological problems. Physical pain can create emotional problems and emotional problems can contribute to physical pain. Once the cycle starts, the emotional and physical problems interact and feed upon each other. Treatment of only one aspect of this problem is often doomed to failure

A large number of women are said to suffer from painful sex at some time or the other in their lives. In majority this remains unspoken and the women takes it that it may be a normal phenomenon or some think that they are alone with this problem. This condition is known as Dysparennia. Some amount of pain during sex is common and goes off after sometime.

Common causes of Pain during sex:

Vaginal Dryness
Vaginismus
Vaginitis
Lichen Sclerosis
Vulvadynia
Vestibulitis
Cystitis
Menopausal Thining
Endometriosis
Pelvic Inflammatory Diseases
Radiation Scarring

Vaginal Dryness - is one of the most common causes of pain during sex especially in young brides because of ignorance, insufficient arousal, inadequate foreplay, fear of pregnancy, stress or hormonal imbalance.
Vaginismus - is a phenomenon in which the muscles at the vaginal opening constrict into tight spasm as soon as intercourse is attempted. This is a protective response against penetration and can happen specially with brides because of extreme fear, inhibition, improper sex education, childhood trauma, witnessing an unpleasant sexual activity. This results in a vicious circle of pain and fear.

Vaginitis - is any type of vaginal infection or may be due to some allergic reaction or chemical irritation. The commonest is Thrush - a fungal infection resulting to foul smelling discharge.

Lichen Sclerosis - is an auto immune condition in which immune cells attack the skin. It can affect any part of body, vulva is a common target. It usually develops after 40.

Vulvadynia - In this condition there are nerve abnormalities around the vulva. It causes constant itching, burning pain around the vulva and vagina which worsens when touched even with water.

Vestibulitis - In this condition too there is burning around vulva with redness and soreness. Tends to affect between 20s-30s.

Cystitis - Most of the women experience this especially on the first initiation into sex. that is why it is also referred to honeymoon cystitis. It may also occur when sex is resumed after prolonged inactivity. It causes burning sensation while urination, frequent cloudy or bloody urine.

Menopausal Thinning - This occurs with menopause when ovaries produce less estrogen resulting to vaginal lining thinning out and shrinking of the vagina.

Endometriosis - In this condition the tissue that lines the uterus gets developed in other areas like fallopian tubes, ovaries abdominal wall or ligaments supporting the uterus. Women in 20s and 30s are most prone to this condition. It causes sharp pain deep in lower abdomen during intercourse. This condition may go after menopause or with hormonal therapy.

Pelvic Inflammation Disease - It can cause following frequent chlamydia infection or infection following childbirth or due to IUD.

Radiation scarring - due to radiation for cervical cancer or endometrial cancer can result in decrease in the length and circumference of the vagina because of adhesions and fibrosis.

Because your friend most likely is young. She needs to relax, use a lot of lubricant and requires a lot of foreplay. And the use of condoms or other forms of birth control will greatly help ease the anxiety and fear of pregnancy. Also, avoid rigorous lovemaking. Patience is very impt. The burning sensation is normal . And dysuria ( painful urination) and blood in urine are very normal conditions after a prolonged sexual inactivity.

If you hadn't had sex for quite sometime, the vagina tends to be constricted again. Upon becoming sexually active again, after a spell of nearly 20 yrs of abstinence; I encountered the same problem. There were so much unbearable pains. Penetration was not made possible. Plus I bled for a few days. Sex was made successful again after lubricant was used judiciously, long periods of foreplay and when I learn to relax and not become too anxious

The burning sensation can be relieved by application of warm compress or douching with warm water. So it's impt to use lubrication to eliminate this pain and make sex more enjoyable.

Hopefully I answered your question thoroughly.

Well first of all, have these past two instances been the first two? I am assuming no, if that is correct it could a number of things. You say it feels like a cut, it is extremely possible that you have torn something internally before the last two times, this would give you sharp pains. Also, does this pain occur only during sex or throughout the day? The bleeding suggests a tear, especially if there are no other signs or symptoms such as nausea, vomiting or fever. Besides a tear there are several other things including a Yeast Infection, Gonorrhea, Polyps and Endometritis. There are less serious reasons such as vaginal dryness after menopause, a common reason for bleeding in women and easily medicated. Any further symptoms would greatly aid in a diagnoses! Whatever it may be bleeding after sex is not normal unless it is your first time. Bleeding after sex requires consultation with a doctor but don't worry too much, may women experience this at least once in their life.

A girl has to have her own natural lubrication going or buy & use some lubricant so sex isn't painful. Most young guys don't care if you're "ready" yet, but next time you need to be sure you use lube. Doing it dry is not the way to go, it'll make you sore for days! Buy some KY or something, get lube. You can excuse yourself and go to the bathroom to use it.

How long ago did she have sex? If it was only a few days ago and she's still in pain, that's normal. If it was, say, a week or more ago... she might want to go to the doctor to get everything checked out. Just in case.

If your friend is in pain and its her first time , its possible that her vaginal muscles arn't use to having it streched so therefore it has damaged her muscles and caused pain.

There is also chances that his penis was to big for her and has hurt her internerly.

Another could be he was to rough or wasnt doing it right.Usually pain can be common after your first sexual experience.

By the way , guys dont usually care about your feelings for your first time and may not know how to care about this also try using some lube , it helps alot.

I hope your friend gets better!!
Goodluck!
