You add a degree when taken axillary (under the arm) and subtract a degree when taken rectally. You do not have to do anything if you are taking it orally.

the temp of mouth is the correct temp you add one if taken in ear or under the arm and subtract one if taken rectally. good luck w/ the little one

No, don't add any degrees, but just make sure you shake the thermometer down to at least 95.9 before you begin...
