It all depends. Garmin and Magellan are selling baseline handheld units for under $100, but if you want color, moving map, and other features you're going to spend a couple times that.
The manufacturer-installed GPS's tend to run in the $1000 range. They offer a nice factory-installed appearance and integrate well, but you can get the same features for less.
No, there isn't a monthly fee. GPS signals are beamed by government satellites and they're free to anyone who can receive them.

Aftermarket systems are available for just several hundres dollars. Higher end ones like Pioneer have the GPS with a DVD player and will cost a couple of thousand.
There is no fee, unless you get the ones that tell you about traffic congestion and where construction is being done.
Mine cost about 800 and has no screen, it is all done by voice activation.

I recently got GPS, and I was wondering about the same exact thing. After doing some research and trying to find the "catch" (which I was sure there was), I was surprized to learn that you only have to pay for the device itself. The GPS signal is free; this is the only thing that could contribute to running-costs if it weren't. Apparently, certain laws declare the GPS satellite signal to be a "public good", much like drinking water, air, etc. At least in the U.S it is. Embrace it while it lasts.

I use my lap top with the MIcrosoft Street's and Trip's with the GPS locater. This is way better than one of those little screens on your dash. It does n't speak to you but you can zoom out see where you are in a city or can zoom in and see a few block area.
I beleive it cost me $130ish
