Most leeches have cryptic colours that allow them to hide from predators when they are lying on the bottom of ponds/lakes.
Leeches can also swim, sometimes quite quickly, but typically their predators are much faster than they are. So, one way in which leeches ensure their survival is to have many offspring. Like many creatures near the bottom of the food chain, having lots of offspring ensures that many will make it to adulthood.
Leeches can also gain added protection by attaching themselves to large animals. Being attached to lets say a snapping turtle or a large northern pike would make them very safe from their predators seeing as most other creatures avoid these animals.

The leeches I've seen are fairly defenseless. Their defenses are to swim away or to drop down to the bottom of the pond and lay still for a long time.
