I have studied AIDS and HIV cases for over 5 years and according to my research the countries with the highest percentage of population infected is Botswana 38.8%, Zimbabwe 33.7%, Lesotho 31%, Namibia 22.5%, South Africa 20.1%, and Zambia 21.5% . This may not interest you, but research has mentioned a variety of factors that may play an influenitial role in the spread of AIDS in a country. These factors include: education rate, religion, migrant workers, poverty, urbanization, and type of government.

Africa is number one. But not because it's so dirty there, like another response mentioned. It's because the people there are so poor and uneducated.
