Not any more unsanitary than flushing the toilet or taking a shower.
They now have screens to place on top of the bath tub to hold the baby in, so it's only the water that goes.

Throwing dirty water out on the dirt doesn't sound unsanitary, just natural...

Not me, how disgusting. I'm a big fan of sanitization. An unsanitary environment is too horrible for words. I can't live without sanitizer; indeed, I have a desperate need for lots of sanitizer in my life. I'm practically insane without sani; I can't sleep until the san man sings. The unsanitized life is not for me.

Yes now it would be, but that was the way it was way back when. However the saying is meant just as "Don't cut off your nose to spite your face." In other words, don't be stupid.
