Slips out of gear when taking off also.

Auto or manual. Auto you need to adjust the shift linkage at the transmission. If it is a manual with original shifter there may not be an adjustment you have a worn out transmission.

Unfortunately, you will have to rebuild your gearbox. At the ends of each shaft is a thrust washer that absorbs the sideways forces that helical gears put on the gearbox casing when accelerating and decelerating. When these wear out, the gears have too much play, and can move far enough from the normal "engaged" position to let the gear disengage. It will usually happen first in the lower gears (first and second) because this is when the greatest torque is applied to the gears, and therefore the greatest side thrust. Removing both shafts and replacing the thrust washers is usually the only way to fix it.
