Unless there is some reason to believe your request is made for fraudulent reasons, it is basically a formality. Adding a middle name should not be a problem. It's not like you can hide from creditors while keeping your existing first and last name.
Alexandria is mistaken about requiring an attorney. The paperwork is fairly straightforward. Someone said it costs $500. That sounded high to me so I searched for "Ohio name change". In Franklin County, OH the cost is $108 + the cost of publishing the application in a "local newspaper of general circulation" for 30 days.

You can simply use a middle name; you don't have to do that legally. If you want to make the change legal, go to the court house and fill out a form for change of name.

Go to your county courthouse, see the clerk, and request info on name changes. It's pretty straight forward, but takes a little time, a court appearance, notice printed in the paper, etc.

To change your name you do have to go to a court house,
and speak to someone who will direct you .
You can change your name or add one or whatever, but
you do have to have a good reason, and also there is
a fee of about $500.00. You also do have to go before a
judge to do it.

I do know that it requires a visit to an attorney. My husband changed his middle name because he hated it.
